AI-Assisted Clinical Decision Support System for Acute Myocardial Infarction and AIoT Remote Application (Smart Heart Platform)

Heart disease is the second leading cause of death in Taiwan and among the top three causes of death in most developed countries worldwide. Acute myocardial infarction (AMI) is particularly severe. According to the National Health Insurance Administration's 2018 statistics, 48,000 people sought medical attention for myocardial infarction that year, and 1 in 8 patients was under 50 years old. For the most critical ST-segment elevation myocardial infarction (STEMI), international medical guidelines recommend that the door-to-balloon time (D2B time) should be within 90 minutes from when the patient arrives at the emergency room to the completion of cardiac catheterization balloon dilation. However, due to some patients' atypical symptoms and the inability to interpret ECGs promptly, the golden treatment opportunity is often missed.

To address this issue, our hospital's Artificial Intelligence Center collaborated with the Cardiovascular Center to develop the "Smart Heart Platform: STEMI Pre-Hospital Remote Diagnostic Assistance System." This system integrates with QT Medical's PCA 500 portable ECG device, allowing emergency medical technicians to perform AI interpretation immediately upon arrival at the scene in the ambulance, and instantly transmit the ECG and analysis results to cardiologists for review, saving precious time in life-saving efforts.

Since June 2021, the "Smart Heart Platform " has been implemented in 52 fire brigades in Taichung and Nantou, assisting over 800 patients in ambulances and successfully identifying 36 critical STEMI cases. The implementation of the AI classification system has significantly reduced D2B time and increased the proportion of cases with in-emergency room D2B times of less than 90 minutes.

Furthermore, our hospital has formed a strategic alliance with the Penghu County Government, integrating the Smart Heart Platform system into the "Chrysanthemum Island Heart Chain" formed by Penghu County's fire departments and health centers, helping frontline medical personnel accelerate emergency notification processes.

The system performs excellently with 99% accuracy, 80% precision, and 99% sensitivity. Research results have been published in international journals, including the Canadian Journal of Cardiology (2021) and European Heart Journal-Digital Health (2021).
