Medication Guidance-Warfarin Tablets 用藥指導-Cofarin(可化凝錠)

  • Product: Cofarin
  • Chinese name: 可化凝錠
  • Content: Warfarin sodium 1mg/5mg
  • Appearance: 1 mg: pink round tab, marked GP//278.
  • 5 mg: pink orange round tab, marked G P// 183 // 5.
  • Target: Anticoagulant that helps prevent blood clots

Usage and dosage

  • Most patients received 2mg to 10mg daily to achieve the desired therapeutic effect. The dose and interval of individual patients should be determined by the international normalized ratio/prothrombin time (INR/PT).
  • The length of treatment should be adjusted according to individual conditions, until the end of the risk for thrombus and embolism.
  • If you forget to take the medicine in the prescribed time, you should take it as soon as possible on the same day. You can't make up for the missed dose by doubling the dosage. If necessary, you should consult a doctor or pharmacist.

Drug storage

  • Store it in an airtight container, at room temperature (15-30 ℃), away from light and humid. Keep it out of the reach of children.

Possible adverse reactions

Inform your physician/pharmacist as soon as possible of any of:

  • Pain, swelling or discomfort, bleeding wounds, increased menstrual flow or vaginal bleeding, nosebleeds, gingival bleeding when brushing teeth, hematuria, bloody stool, bruising, purple spot, headache, dizziness or weakness.

Special medication instructions

  • Please follow the directions and dosage given by your physician/pharmacist, preferably at the same time.
  • Do not change the dosage or discontinue the drug without instructions from the physician/pharmacist.
  • Please regularly measure the INR/PT and relevant coagulation tests as directed by your physician.
  • Please let your physician/pharmacist and dentist know that you are taking Cofarin.
  • If you are about to undergo surgery or tooth extraction, ask physician/pharmacist if you need to stop Cofarin and when to restart it.
  • Pregnant women, women planning to become pregnant or lactating women are advised to inform their physician/pharmacist before taking the drug.
  • Avoid sports and other activities that may injure you.

Possible interactions between food and warfarin

Lowering anticoagulation

Enhancing anticoagulation

  • Vitamin K (green vegetables eq: Spinach, broccoli, sweet potato leaves, green onions (scallions), cilantro, Thai basil, avocado, etc)
  • Animal organs (e.g., beef liver, pork liver)
  • Green tea* Black tea* soymilk (soybeans)*
  • Chickpeas (garbanzo beans), soybean products (e.g., firm tofu, dried tofu), and natt
  • Vitamin C(>1-2g/day)
  • Coenzyme Q10, St.John’s wort
  • Ginseng
  • Alcohol (large amounts)
  • Large amounts of garlic, onions, chili peppers, ginger, and ginger powder (small amounts used in cooking have less impact)
  • Large amounts of grapefruit, pomelo, papaya, cherries, mangoes, cranberries, and pomegranates (including processed products such as juices, dried fruits, and jams).
  • Chitin*, fish oil (>2g/天), vitamin E (> 400 IU/day), Glucosamine (>500mg/
  • day), Pineapple enzyme, royal jelly, grape seeds
  • TCM: Ginkgo biloba, Angelica sinensis, Salvia miltiorrhiza, Carthamus tinctorius, Crocus sativus, ginseng

*Only a few cases have been reported or supported by animal experiments.
