Why is a Sitz Bath Necessary?

During cancer treatment, a patient's immune system is often suppressed. Diarrhea or constipation caused by chemotherapy and radiation therapy can easily irritate the anal and perineal areas. Because this region has many skin folds, it is prone to trapping dirt and moisture, creating an ideal environment for bacterial growth.

Regular sitz baths promote local blood circulation, relax muscles, alleviate pain, and protect the mucosal lining from bacterial infections, improving overall patient comfort.

Equipment Needed

Sitz bath basin, towel, povidone-iodine solution, and warm water (boiled water that has cooled down is preferred).

Step-by-Step Procedure

  1. Prepare the Solution: Fill the sitz bath basin about halfway with warm water. Add povidone-iodine solution at a ratio of 1:100 (e.g., add 10 mL of povidone-iodine to 1,000 mL of water).
  2. Test the Water Temperature: The water should feel comfortably warm without causing burns (approximately 41°C–43°C). Test the temperature using a water thermometer or the inside of your wrist.
  3. Immerse Fully: Ensure the anal and perineal areas are completely submerged in the water.
  4. Frequency and Duration: Perform the sitz bath twice daily (morning and evening) and after every bowel movement, soaking for 15–20 minutes each time.
  5. Ensure Comfort and Privacy: Place both feet flat on the floor during the sitz bath to prevent pressure on the thighs, tailbone (coccyx), and sacrum. Drape a bath blanket over the patient's thighs for warmth and privacy.
  6. Pat Dry Gently: After the soak, gently pat the buttocks dry with a clean, soft towel. Do not rub vigorously to avoid skin breakdown or abrasion.

Important Precautions

  • Maintain Schedule: Perform sitz baths regularly as scheduled to achieve optimal anti-inflammatory and soothing effects.
  • Keep Basin Clean: Rinse the sitz bath basin thoroughly with clean water before and after every use.
  • Mild Cleansing: When washing the perianal area during routine hygiene, use a mild, non-irritating soap.
  • Protect the Mucosa: Maintain regular bowel habits to prevent diarrhea or constipation from injuring the perianal skin and mucosa.
  • Prevent Burns: When adjusting water temperature, always add cold water first, followed by hot water.
  • Safety Warning Signs: If symptoms such as nausea, dizziness, extreme weakness, or rapid pulse occur, stop the sitz bath immediately and notify healthcare staff.
