Procedures Child Development Delay Reporting

1. Apply for medical examination certificate (comprehensive evaluation report/diagnosis certificate).	2. Visit the “Ministry of Health and Welfare Social and Family Affairs Administration – Developmental Delay Child Reporting and Case Management Service Center Service Site” and search for “Developmental Delay Child Reporting” or scan the “QRCODE”
3. Click “Online Reporting.”	4. Select “General Public”
5. Complete all data on the “Suspected Developmental Delay Child Reporting Form” and enclose the medical examination report.	6. After completing the report, call the Taichung City Child Development Community Center of the domicile to verify the data.

• Acquire the reporting on the completion of medical examination certificate and apply for early intervention subsidy after taking the early intervention class.
• Receive the “Early Intervention Subsidy Manual” at the district office.
• Transportation fee: NT$200 per travel per person. Up to 12 travels per month for average households and unlimited travels for low-income households.
• Intervention Fee: Apply for the amount of actual expense on the printed invoices.

Child Development Community Centers

Services: Parenting consultation, parenting illustration, parent-child group activities, parent-child game room, books and teaching materials borrowing, to-door services. Please consult with all offices for more information.

Taichung City Child Development Resource Center

• Target: Families having 0-6 years old children at home in Taichung.
• Service Hours: Tuesday – Saturday 08:00A.M. -17:00P.M.
• Services:
• Parent-Child Services: Parent-child home activity courses for 0-3 years old, community reading and game room.
• Consulting Services: children development related intervention and consultation for 0-6 years old.
• Family Services: family psychological support services, study and educational reading.
• Community Services: Community promotion, seminar course, care development guide.

Parent Education Program

• Target: Parents or parents-to-be with 0-2 years old infants at home in Taichung City.
• Service Hours: Monday – Friday 09:00A.M. -17:00P.M.
• Services:
• Parenting Consultation: Parents with infants under 2 years old can call to consult care questions.
• Parenting Practice Classroom: Parents learn parenting skills in taking care of infants with emphasis on practice and drills.
• To-door Parenting Instruction Services: Parenting advisor provide to-door services to help parents learn the skills in taking care of children.

Taichung City Parent Child Center and Mini Parent-Child Center

• Service Hours: Tuesday to Sunday, 9:00A.M. to 17:00 P.M.
• Target: Preschool children under 6 years old and their caregivers.
• Services: Center games, parent-child activities, parenting education, promotional activities, parenting consultation, resource links, and development screening.
• Registration: Limited number of groups per time slow for admission. Register via phone or online.
