The "AD-8 Early Dementia Screening Questionnaire" is a quick and simple quiz designed to help you spot the very early signs of dementia.

How does it help you? By simply checking the boxes that apply, it helps you or your family members keep track of any changes that have happened over the past few years. Catching these changes early means you can get a doctor's evaluation much sooner.

AD-8 Early Dementia Screening Questionnaire

Instructions: Please think back over the last few years and see if you've noticed any of the following situations. Focus on changes: We are looking for things that didn't happen in the past, but have recently started occurring or have become much more noticeable.
